Exercise should be 'standard part of cancer care

All patients getting cancer treatment should be told to do two and a half hours of physical exercise every week, says a report by Macmillan Cancer Support.

Theo báo cáo của Trung tâm hỗ trợ ung thư Macmillan, tất cả bệnh nhân đang điều trị ung thư cần được khuyến khích luyện tập thể thực khoảng 2 tiếng rưỡi mỗi tuần.

Being advised to rest and take it easy after treatment is an outdated view, the charity says.

Research shows that exercise can reduce the risk of dying from cancer and minimise the side effects of treatment.

The Department of Health says local initiatives can get people moving.

Macmillan's report, Move More, says that of the two million cancer survivors in the UK, around 1.6 million are not physically active enough.

Adult cancer patients and cancer survivors should undertake 150 minutes of moderate intensity physical activity per week, the reports says, which is what the Department of Health guidelines recommend.

In the report, the American College of Sports Medicine also recommends that exercise is safe during and after most types of cancer treatment and says survivors should avoid inactivity

It doesn't need to be anything too strenuous, doing the gardening, going for a brisk walk or a swim, all count”

Bạn không cần phải luyện tập quá gắng sức, làm vườn, đi bộ nhanh hoặc bơi, đều có tác dụng.”

Getting active, the report says, can help people overcome the effects of cancer and its treatments, such as fatigue and weight gain.

"The evidence review shows that physical exercise does not increase fatigue during treatment, and can in fact boost energy after treatment."

"It can also lower their chances of getting heart disease and osteoporosis.

“Nó đồng thời làm giảm nguy cơ mắc bệnh tim mạch và loãng xương.”

"Also, doing recommended levels of physical activity may reduce the chance of dying from the disease. It may also help reduce the risk of the cancer coming back."

Previous research shows that exercising to the recommended levels can reduce the risk of breast cancer recurring by 40%. For prostate cancer the risk of dying from the disease is reduced by up to 30%.

Bowel cancer patients' risk of dying from the disease can be cut by around 50% by doing around six hours of moderate physical activity a week.

Nguy cơ tử vong ở những bệnh nhân ung thư ruột có thể giảm khoảng 50% với việc luyện tập vừa sức khoảng 6 tiếng mỗi tuần.

Ciaran Devane, chief executive of Macmillan Cancer Support, said physical activity was very important to the survival and recovery process.

Ciaran Devane, Giám đốc Trung tâm Hỗ trợ ung thư Macmillan, nói rằng luyện tập thể lực là vô cùng quan trọng để sống sót và hồi phục.

Keeping active after treatment for cancer is now recommended by cancer experts

"Cancer patients would be shocked if they knew just how much of a benefit physical activity could have on their recovery and long term health, in some cases reducing their chances of having to go through the gruelling ordeal of treatment all over again.

“Bệnh nhân ung thư có thể sửng sốt nếu họ biết những lợi ích mà hoạt động thể lực mang lại cho sự hồi phục và sức khỏe lâu dài của họ, trong một số trường hợp, nó làm giảm sự số lần điều trị mệt mỏi mà bệnh nhân phải chịu đựng liên tục.

Traditionally cancer patients were told to rest after their cancer treatment, but the report says this approach could put cancer patients at risk.

Jane Maher, chief medical officer of Macmillan Cancer Support and a leading clinical oncologist said: "The advice that I would have previously given to one of my patients would have been to 'take it easy'.

"This has now changed significantly because of the recognition that if physical exercise were a drug, it would be hitting the headlines."

Martin Ledwick, head information nurse at Cancer Research UK, was a little more cautious.

Martin Ledwick, Y tá trưởng Trung tâm Nghiên cứu Ung thư Vương Quốc Anh, thận trọng hơn

"Anything that improves wellbeing and reduces treatment side effects for cancer survivors has to be a good thing

“Bất cứ điều gì cải thiện sức khỏe và làm giảm tác dụng phụ của quá trình trị liệu cho bệnh nhân ung thư thì đều tốt”

"But the evidence that exercise has a bearing on survival is not conclusive. It is important to remember that no two cancer patients are the same, so rehabilitation programmes that include physical activity will need to be tailored to the individual."

A spokesperson from the Department of Health said it was vital that people with cancer are given the support to lead an active life.

"Physical activity and a healthy lifestyle can impact very positively on cancer outcomes and, as part of the National Cancer Survivorship Initiative, we are working with Macmillan to integrate physical activity services into cancer care pilot sites.

"Locally led initiatives such as Let's Get Moving are also well placed to signpost cancer patients to community-based physical activity opportunities."

A healthy dose of exercise is good medicine, even for lymphoma patients receiving chemotherapy, says Kerry Courneya, Canada Research Chair in Physical Activity and Cancer in the Faculty of Physical Education and Recreation at the University of Alberta.

Thể dục vừa sức là liều thuốc tốt, ngay cả đối với bệnh nhân ung thư máu đang sử dụng hóa trị, theo lời Kerry Courneya, Chủ tịch Trung tâm nghiên cứu Hoạt động thể chất với bệnh ung thư, Khoa Giáo dục thể chất và Giả trí thuộc Đại học Alberta, Canada.

The Healthy Exercise for Lymphoma Patients trial, a three-year study led by Courneya, published last month in the Journal of Clinical Oncology, found that a regimen of aerobic exercise training produced significant improvements in physical functioning and overall quality of life benefits in patients with lymphoma.

Researchers recruited 122 patients with Hodgkin's and non-Hodgkin's lymphoma, then classified participants by disease type and treatment status, whether they were undergoing chemotherapy at the time or receiving no treatments. Participants were randomly assigned to an exercise program designed to maximize cardiovascular fitness or to usual care, which did not include an exercise component.

"The exercise program consisted of interval training," said Courneya. "We had patients ride the bike at a modest intensity, interspersed with high-intensity bouts of exercise, where they would go full out, exerting maximum effort for a minute or two at a time, then rest for a few minutes before doing it again. That type of interval training has really been shown to maximize improvements in fitness."

Exercisers trained three times a week for 12 weeks and were encouraged to stay the course with behavioural support techniques that included perks like free parking, a well-equipped gym, flexible exercise schedules, variation in exercises, follow-up phone calls reminders and positive reinforcement by staff.

Lymphoma patients who received the exercise intervention reported significantly improved physical functioning, overall quality of life, less fatigue, increased happiness, less depression and an improvement in lean body mass. In fact, cardiovascular fitness in the exercise group improved by over 20 per cent.

“That's considered a fairly large improvement over a 12-week period," says Courneya, adding that the group receiving chemotherapy benefited as much as the group that was off treatments.

"That's important because we know that fitness improvements are related to improvements in how cancer patients feel both functionally as well as emotionally," said Courneya.

“Điều này quan trong bởi chúng ta đều biết cải thiện tình trạng sức khỏe liên quan đến sự cải thiện về cách bệnh nhân cảm thấy như thế nào cả về cả khả năng hoạt động lẫn cảm xúc”, Courneya nói.

Courneya also found that the vigorous intensity exercise program did not interfere with lymphoma patients' ability to complete their chemotherapy treatments or benefit from the treatments.

"The improvements in fitness and in how they felt were really important. The most important finding from a safety perspective is that the patients were able to complete their chemotherapy as scheduled," he says.

“Sự cải thiện tình trạng sức khỏe và việc bệnh nhân cảm thấy như thế nào là vô cùng quan trọng. Phát hiện quan trọng nhất từ quá trình đánh giá là bệnh nhân hoàn toàn có khả năng hoàn thành đợt hóa trị liệu như dự kiến”, ông nói.

"In addition, we found some suggestion that the group that did the exercise had a better response to their treatment. In the exercise group we found that 46 per cent of patients had a 'complete response.' That means the tumour has gone and there's no evidence of disease, compared to only about 30 per cent in the usual care group.

"The study wasn't really designed to look at that but it's a very provocative finding and suggests that perhaps this type of exercise training program during treatment might allow patients to respond better to the treatments and get better disease control."

Initiatives like Wellspring, which is establishing cancer patient support groups all over the country, and organizations like the Canadian Cancer Society, the YMCA and others, are forging ahead in developing supportive care interventions that include exercise programs.

"Ultimately," says Courneya, "it's important that we get the information out to those groups, so we can inform cancer patients, and help them access this type of exercise program."

Carol Vorderman says pupils should study maths to 18 - Học sinh Anh nên học toán tới 18 tuổi

Carol Vorderman says pupils should study maths to 18
School pupils in England should study maths up to the age of 18, a report commissioned by the government says.

It says radical change is needed to give children the mathematical skills needed to succeed in a workplace where numeracy is increasingly important.

The report, by TV presenter Carol Vorderman, said the current system was failing young people.

Almost half of 16-year-olds fail to achieve grade C at GCSE, with 15% studying maths beyond that level.

This compares to a rate of 100% in most industrialised nations.

Ms Vorderman said more than 300,000 16-year-olds each year completed their education without enough understanding of maths to function properly in their work or private lives.

She said 24% of economically active adults were "functionally innumerate", and universities and employers complained that school-leavers did not have necessary maths skills.

Ms Vorderman told BBC Radio 4's Today programme that pupils who did not achieve the expected standard - level 4 - in the national curriculum tests known as Sats at age 11, faced a "catastrophe".

Some 90% of them go on to fail to get a C at GCSE, she said.

"If you're on the scrap heap by 11, you will remain mathematically on the scrap heap," she said.
Split exams

Ms Vorderman led a "maths task force" to produce the report, which was commissioned by Mr Gove and Prime Minister David Cameron when they were in opposition in 2009.

Her team concluded that the GCSE curriculum leans towards advanced topics needed by those who will study maths at A-level, which puts off less-gifted pupils.

Ms Vorderman said pupils were being taught trigonometry and algebra when "they can't even calculate a percentage".

The report recommends that the current maths GCSE should be split into two separate exams.

One would offer a higher standard of education in the core areas of the curriculum, such as basic numeracy and personal finance, while the other acted as a preparation for A-level.

The report says all pupils should study maths to 18, but this should not necessarily be in the form of an A-level or AS-level course, but should include a range of options to suit all abilities.

The task force also said that many primary school teachers are not adequately prepared to teach the subject and staff shortages mean a quarter of secondary pupils are taught by non-specialist maths teachers.

The report calls for better training to improve primary teachers' subject knowledge and confidence; the active encouragement of maths activities outside the daily lesson; and a new assessment for 11-year-olds to replace Sats.

Ms Vorderman said: "Mathematics is a critically important subject. It is a language without which the entire global infrastructure is struck dumb.

"This report does not make comfortable reading. It is aspirational but this does not mean making maths "harder" for everyone; it means making the teaching better and what is taught much more suitable for those who are learning it."

In the face of current financial turmoil, Ms Vorderman said: "Who knows which countries will come out on top in 20 years - is it going to be a country which has a lot of numerate people, or is the one that doesn't bother?"

In June, Education Secretary Michael Gove said he would like to see the "vast majority" of pupils in England studying maths to the age of 18 within a decade.

He said there were strong arguments for "making certain subjects compulsory for longer".

Mr Gove said: "This comprehensive report, looking at all the important areas, will be of great help as the government continues its drive to equip our children with the skills that they need to compete with their global contemporaries and thrive in the 21st Century."

An investigation into how Vietnamese children trafficked to work in the UK's cannabis trade are prosecuted as criminals.
Children of the cannabis trade investigates how Vietnamese children forced to work underground in the booming cannabis trade, held hostage by debt and poverty, are often prosecuted as criminals rather than victims of trafficking when discovered.
With the Vietnamese government now announcing a nationwide push for export labour, and the increasing demand for home-grown cannabis in the UK, the exploitation of Vietnamese children for criminal profit in the drugs industry is a disturbing trend that shows no signs of abating.
Here Mei-Ling McNamara, the producer of this episode of People & Power, writes about the making of Children of the cannabis trade.
For almost a decade, police in Britain have been struggling to cope with an explosion of criminality connected to the country's flourishing but illegal cannabis trade. This has grown rapidly in recent years because of a huge rise in the number of secret indoor cannabis farms - hidden away in suburban dwellings and disused premises across the country. As the authorities shut down more of these operations every year, so more spring up in their place - many of them tied into an expanding network of organised crime, corruption and violence.
Back in 2004, the police made an especially sinister discovery about the trade - that Vietnamese children and teenagers were being trafficked across the world for use as slave labour in the farms. Remarkably, Vietnamese crime gangs run many of the illegal cannabis operations in the UK and often use children - exploitable because their families are in debt bondage to moneylenders in their native country - to work on a production process that exists to meet spiralling demand for the drug on the streets of Britain.
The cannabis farms can be extraordinarily profitable, but little if any money is lavished on the premises or on the conditions under which the children toil. Set up in private residences or industrial sites, often gutted for the purposes, the operations can involve thousands of indoor cannabis plants.
Boys and girls - some as young as 13, many not older than 16, are forced to work as 'gardeners', trapped inside the buildings, 24 hours a day, tending and watering the plants behind blacked-out windows with no ventilation. Eating, sleeping and working under heat lamps and exposed daily to toxic chemicals, they run a constant risk of electrocution and fire. And all the time they face the violence, intimidation and extortion of gang members who are determined to wring everything out of them until their debts are paid off - if that day ever comes.
But when the police identify and raid the premises - and such raids are increasingly common - the plight of these young people is far from over. More often than not they are treated as offenders in the narcotics business, rather than as potential victims of trafficking.
Moreover, as many of them are psychologically disturbed by the emotional and physical trauma they have experienced, they are often terrified of revealing their stories to the police - not least because of fears that if they talk, their family members back in Vietnam will be punished for their failure to pay off outstanding debts owed to moneylenders connected to the gangs.
A vicious cycle
Vòng luẩn quẩn
Vietnamese children now make up the largest group of children being trafficked into the UK, primarily for exploitation in the cultivation of cannabis. According to the UK government's CEOP organisation (Child Exploitation and Online Protection Centre), nearly 300 children per year are trafficked into the country - and nearly a quarter can end up on cannabis farms.
If they are recovered by authorities they are under extreme pressure to abscond from care, with traffickers often making threats. Once bailed or released from custody, nearly two-thirds of Vietnamese children go missing from local authority care soon after.
According to anecdotal reports from care advisers, some are re-trafficked and return to a new cannabis farm, while others go back to their traffickers to pay off debts and avoid deportation. The threat of violence against a child or their family members is used as a powerful tool to ensure cooperation.
Many Vietnamese minors have been charged, prosecuted and sentenced for the production and supply of cannabis, but only 58 children last year were deemed trafficked when found in these environments. And to date, there have been no known convictions of Vietnamese criminals who have trafficked children into the UK for the purpose of cannabis cultivation.
Networks may specifically recruit children, as they are less likely to be detained or accommodated in secure premises then adults and are able to re-enter exploitation with relative ease.
UK's cannabis explosion
Ten years ago, only 11 per cent of the marijuana used in the UK was grown domestically. Now that figure has grown to nearly 90 per cent. The cannabis trade is so lucrative, authorities say, that they fear the mass surplus is even being exported to EU countries.
Last year, UK law enforcement uncovered 1.3 million cannabis plants worth an estimated $410m. One house can produce cannabis worth up to $500,000 or more a year and during 2010, the police found nearly 7,000 factories during raids - the number has increased by 900 per cent in the past six years.
The UK authorities estimate that 75 per cent of the criminal gangs involved in this trade are ethnically Vietnamese, although local British and Eastern European gangs are beginning to muscle in. However, research has found that even in these circumstances, Vietnamese 'gardeners' are still used to cultivate the cannabis plants because they are sold on to or taken over by the incoming gang.
Trafficking victims have been found in all regions of England and Wales - most frequently in West Yorkshire, the West Midlands and Greater London. Now some are being discovered in Scotland, and cannabis factories have also been identified in Northern Ireland.
Although an increasing number of police forces are identifying Vietnamese cannabis farms locally, the ability and ease with which networks can relocate at a national level (including the relocation of trafficked children) and thereby evade the police, ensures that the number of children trafficked and exploited in cannabis farms is likely to remain at a high level.
Coercion, recruitment and debt bondage
Vietnam is one of the poorest countries in south-east Asia, and the country is heavily reliant on an estimated $2bn worth of remittances paid by Vietnamese workers overseas. Last year, nearly 100,000 migrants went abroad for work.
In these circumstances the door is wide open for exploitation, both by illegal labour agencies and traffickers posing as potential recruiters for overseas employers. It is not uncommon for Vietnamese labour export companies, most of which are state-affiliated, to charge workers well in excess of the fees allowed by law, sometimes demanding as much as $20,000 up front for the opportunity to work abroad.
Paying such sums back is extraordinarily difficult and Vietnamese expatriate workers and economic migrants are consequently highly vulnerable to debt bondage and forced labour. On arrival in destination countries, many workers find themselves compelled to work in dangerous or substandard conditions for little or no pay with no credible avenues of legal recourse. When the work itself is illegal, as is often the case, then the authorities are the last people to whom the workers can turn for help.
Debt bondage is common, with the trafficking and criminal networks determining the amount of money the bonded worker will have to pay off through unpaid labour. The debt notionally covers travel arrangements, accommodation, food and trafficker fees, but the sums are often inflated and can take several years to work off. In the UK, debt bondage sums have been found to range from between $25,000 and $60,000.
In Vietnam, traffickers, often posing as 'middlemen' for the export labour market, will target isolated children or vulnerable families living in relative poverty. They may make false promises about a better life for the child in the UK, with the opportunity of education or work for the child so that they can support themselves or their relatives back home. A debt will often be placed on the child or their family that cannot afford the travel costs, often secured against a relative's land.
Some of the victims are sent to Russia with fake ID cards and then travel to the Czech Republic, Germany and France, entering the UK by clandestine methods via a seaport. Upon arrival, they are ripe for exploitation by the gangs who bring them straight to cannabis factories. They usually know their families back home and are aware of the debt that must be paid off.
The criminal networks involved in the recruitment, transportation and exploitation of the children are well organised, flexible and generate large sums mainly from the cultivation and wholesale distribution of cannabis.
Agents often provide travel documents but then take these documents off the children once they have been used, recycling them for use with other children. Agents trafficking Vietnamese victims often take back or instruct the child to destroy documentation before entering the UK. Without documentation, it is difficult to question the true identity, age and origin of a child, preventing or delaying removal and protecting the traffickers, thus keeping their trade underground.

Secret Desert Force Set Up by Blackwater’s Founder

ABU DHABI, United Arab Emirates— Late one night last November, a plane carrying dozens of Colombian men touched down in this glittering seaside capital. Whisked through customs by an Emirati intelligence officer, the group boarded an unmarked bus and drove roughly 20 miles to a windswept military complex in the desert sand.

Sheik Mohamed bin Zayed al-Nahyan of Abu Dhabi hired Erik Prince to build a fighting force.

The Colombians had entered the United Arab Emirates posing as construction workers. In fact, they were soldiers for a secret American-led mercenary army being built by Erik Prince, the billionaire founder ofBlackwater Worldwide, with $529 million from the oil-soaked sheikdom.

Mr. Prince, who resettled here last year after his security business faced mounting legal problems in the United States, was hired by the crown prince of Abu Dhabi to put together an 800-member battalion of foreign troops for the U.A.E., according to former employees on the project, American officials and corporate documents obtained by The New York Times.

The force is intended to conduct special operations missions inside and outside the country, defend oil pipelines and skyscrapers from terrorist attacks and put down internal revolts, the documents show. Such troops could be deployed if the Emirates faced unrest or were challenged by pro-democracy demonstrations in its crowded labor camps or democracy protests like those sweeping the Arab world this year.

The U.A.E.’s rulers, viewing their own military as inadequate, also hope that the troops could blunt the regional aggression of Iran, the country’s biggest foe, the former employees said. The training camp, located on a sprawling Emirati base called Zayed Military City, is hidden behind concrete walls laced with barbed wire.

Photographs show rows of identical yellow temporary buildings, used for barracks and mess halls, and a motor pool, which houses Humvees and fuel trucks. The Colombians, along with South African and other foreign troops, are trained by retired American soldiers and veterans of the German and British special operations units and the French Foreign Legion, according to the former employees and American officials.

Mexico City (CNN) -- Mexican authorities said they found and detained more than 500 illegal immigrants from Central America and Asia who were crammed inside two tractor-trailers heading toward the United States on Tuesday.

X-ray equipment detected the immigrants at checkpoints in the southern state of Chiapas, the state attorney general's office said. The drivers of both trucks sped away after authorities ordered them to stop, but police apprehended them nearby.

Inside the tractor-trailers, they found 513 people from El Salvador, Ecuador, China, Japan, Guatemala, India, Nepal, Honduras and the Dominican Republic.

Each of them had paid $7,000 to travel to the United States, the attorney general said.

They had been traveling in "inhumane conditions," Mexico's National Migration Institute said.

Officials said the group was receiving food, shelter and medical assistance while Mexican federal immigration authorities reached out to consulates and embassies to arrange their return to their home countries.

The southern border state of Chiapas, which borders Guatemala, is a major crossing point for Central American immigrants heading north.

In 2010, Mexican authorities apprehended about 25,000 illegal immigrants there, according to statistics from Mexico's National Migration Institute.
