After a decade of painstaking research, we have concluded that American firms are on average the best managed in the world. This is not what we — a group of European researchers — expected to find. But while Americans are bad at football (or soccer, as it's known as locally), they are the Brazilians of Management.

Over the past decade, a team from Harvard Business School, London School of Economics, McKinsey & Company, and Stanford has systematically surveyed global management. We have developed a tool to measure management practices across operational management, monitoring, targets, and people management. We scored each dimension on a range of practices to generate an overall management score, surveying over 10,000 firms in 20 countries. This has allowed us to create the first global database of management practices.

Here are some of our findings.

Well managed firms thrash their poorly managed competitors

First, not surprisingly, we find that organizations with better management massively outperform their disorganized competitors. They make more money, grow faster, have far higher stock market values, and survive for longer. (For details see our previous HBR blog post.)

The American Management Century

Second, when it comes to overall management, American firms outperform all others. This U.S. dominance occurs in the manufacturing, retail, and healthcare sectors (but interestingly, not in high schools). Japanese, German, and Swedish firms follow closely behind. In contrast, developing countries like Brazil, China, and India lag at the bottom of the management charts. Southern European countries like Portugal and Greece appear to have management practices barely better than those of most developing countries. In the middle stand countries like the UK, France, Italy, and Australia, which have reasonable but not brilliant management practices.

Bottom dwellers drive the rankings down

While the ranking of countries is certainly eye-catching, the real story lies within the countries. Almost 90% of the cross-country differences are driven by the size of the "tail" of really badly managed firms within each country. Countries like the U.S. that excel have hardly any badly managed firms, while those like India that have low average scores have a mass of very badly managed firms pulling down their averages.

Every country has some world-class firms

But while there are many of these extremely badly managed, every country also hosts some excellent firms. Even bottom-ranking India has dozens of firms that use world-class management practices. A key takeaway is that individual companies are not trapped by the national environments in which they operate — there are top performers in all countries surveyed. Conversely, being in a world-class environment like the U.S. does not guarantee success. Even in America, more than 15% of firms are so badly managed that they are worse than the average Chinese or Indian firm.

What is the secret sauce of management success?

One of the biggest drivers of these differences is variation in people management. American firms are ruthless at rapidly rewarding and promoting good employees and retraining or firing bad employees. The reasons are threefold.

Many developing-country firms, even while trying to implement new techniques like Lean Management, ignore the fact that labor is different from other "inputs." Many of the Chinese firms surveyed did not even employ managers who spoke the same language as the workers, relying on interpreters or basic sign-language for communication. As you can imagine, this does not lead to a feeling of mutual support between management and workers.

But the U.S. should not be complacent. Other countries equal or better the U.S. in some of the other areas of management we examined, such as careful monitoring, lean production, and sensible targets. The manufacturing prowess of Germany, which has helped it weather the recent downturn so well, is built upon such advantages. Furthermore, although Chinese management practices are well below U.S. standards, they showed the fastest improvement since 2006 of any country we have looked at.

Changing the ranks and reaping the rewards

What lessons emerge for others wanting to reach the top of the ranking?

The answer is not for all firms to be more American but rather to consider some of the practices U.S. firms — and especially U.S. multinationals — continually exhibit and implement. Across all countries, organizations that properly incentivize talented workers, whether through promotion, pay, or other rewards, outperform others. As best practices spread and firms continue to implement these techniques they will narrow the existing gaps, reaping huge growth and profitability gains.

For the first time, Massachusetts General Hospital (MGH) researchers have isolated egg-producing stem cells from the ovaries of reproductive age women and shown these cells can produce what appear to be normal egg cells or oocytes. In the March issue of Nature Medicine, the team from the Vincent Center for Reproductive Biology at MGH reports the latest follow-up study to their now-landmark 2004 Nature paper that first suggested female mammals continue producing egg cells into adulthood. "The primary objective of the current study was to prove that oocyte-producing stem cells do in fact exist in the ovaries of women during reproductive life, which we feel this study demonstrates very clearly," says Jonathan Tilly, PhD, director of the Vincent Center for Reproductive Biology in the MGH Vincent Department of Obstetrics and Gynecology, who led the study. "The discovery of oocyte precursor cells in adult human ovaries, coupled with the fact that these cells share the same characteristic features of their mouse counterparts that produce fully functional eggs, opens the door for development of unprecedented technologies to overcome infertility in women and perhaps even delay the timing of ovarian failure."

The 2004 report from Tilly's team challenged the fundamental belief, held since the 1950s, that female mammals are born with a finite supply of eggs that is depleted throughout life and exhausted at menopause. That paper and a 2005 follow-up published in Cell showing that bone marrow or blood cell transplants could restore oocyte production in adult female mice after fertility-destroying chemotherapy were controversial; but in the intervening years, several studies from the MGH-Vincent group and other researchers around the world have supported Tilly's work and conclusions.

These supporting studies include a 2007 Journal of Clinical Oncology report from the MGH-Vincent team that showed female mice receiving bone marrow transplants after oocyte-destroying chemotherapy were able to have successful pregnancies, delivering pups that were their genetic offspring and not of the marrow donors. A 2009 study from a team at Shanghai Jiao Tong University in China, published in Nature Cell Biology, not only isolated and cultured oocyte-producing stem cells (OSCs) from adult mice but also showed that those OSCs, after transplantation into the ovaries of chemotherapy-treated female mice, gave rise to mature oocytes that were ovulated, fertilized and developed into healthy offspring.

"That study singlehandedly deflated many of the arguments from critics of our earlier Nature paper by showing that oocyte-producing stem cells exist in mice and could develop into fully functional eggs," says Tilly. Another paper from a west-coast biotechnology company, published in Differentiation in 2010, provided further independent confirmation of Tilly's earlier conclusions regarding the presence of oocyte-producing stem cells in ovaries of adult mice.

Tilly is quick to point out, however, "These follow-up studies, while providing definitive evidence that oocyte-producing stem cells exist in ovaries of adult female mammals, were not without their limitations, leaving the question open in some scientific circles of whether the adult oocyte pool can be renewed. For example, the protocol used to isolate OSCs in the 2009 Nature Cell Biology study is a relatively crude approach that often results in the contamination of desired cells by other cell types." To address this, the MGH-Vincent team developed and validated a much more precise cell-sorting technique to isolate OSCs without contamination from other cells.

The 2009 study from China also had isolated OSCs based on cell-surface expression of a marker protein called Ddx4 or Mvh, which previously had been found only in the cytoplasm of oocytes. This apparent contradiction with earlier studies raised concerns over the validity of the protocol. Using their state-of-the-art fluorescence-activated cell sorting techniques, the MGH-Vincent team verified that, while the marker protein Ddx4 was indeed located inside oocytes, it was expressed on the surface of a rare and distinct population of ovarian cells identified by numerous genetic markers and functional tests as OSCs.

To examine the functional capabilities of the cells isolated with their new protocol, the investigators injected green fluorescent protein (GFP)-labeled mouse OSCs into the ovaries of normal adult mice. Several months later, examination of the recipient mouse ovaries revealed follicles containing oocytes with and without the marker protein. GFP-labeled and unlabeled oocytes also were found in cell clusters flushed from the animals' oviducts after induced ovulation. The GFP-labeled mouse eggs retrieved from the oviducts were successfully fertilized in vitro and produced embryos that progressed to the hatching blastocyst stage, a sign of normal developmental potential. Additionally, although the Chinese team had transplanted OSCs into ovaries of mice previously treated with chemotherapy, the MGH-Vincent team showed that it was not necessary to damage the recipient mouse ovaries with toxic drugs before introducing OSCs.

In their last two experiments, which Tilly considers to be the most groundbreaking, the MGH-Vincent team used their new cell-sorting techniques to isolate potential OSCs from adult human ovaries. The cells obtained shared all of the genetic and growth properties of the equivalent cells isolated from adult mouse ovaries, and like mouse OSCs, were able to spontaneously form cells with characteristic features of oocytes. Not only did these oocytes formed in culture dishes have the physical appearance and gene expression patterns of oocytes seen in human ovaries -- as was the case in parallel mouse experiments -- but some of these in-vitro-formed cells had only half of the genetic material normally found in all other cells of the body. That observation indicates that these oocytes had progressed through meiosis, a cell-division process unique to the formation of mature eggs and sperm.

The researchers next injected GFP-labeled human OSCs into biopsied human ovarian tissue that was then grafted beneath the skin of immune-system-deficient mice. Examination of the human tissue grafts 7 to 14 days later revealed immature human follicles with GFP-negative oocytes, probably present in the human tissue before OSC injection and grafting, as well as numerous immature human follicles with GFP-positive oocytes that would have originated from the injected human OSCs.

"These experiments provide pivotal proof-of-concept that human OSCs reintroduced into adult human ovarian tissue performed their expected function of generating new oocytes that become enclosed by host cells to form new follicles," says Tilly, a professor of Obstetrics, Gynecology and Reproductive Biology at Harvard Medical School and chief of Research at the MGH Vincent Department of Obstetrics and Gynecology. "These outcomes are exactly what we see if we perform the same experiments using GFP-expressing mouse OSCs, and GFP-expressing mouse oocytes formed that way go on to develop into fully functional eggs.

"In this paper we provide the three key pieces of evidence requested by those who have been skeptical of our previous work," he adds. "We developed and extensively validated a cell-sorting protocol to reliably purify OSCs from adult mammalian ovaries, proving once again that these very special cells exist. We tested the function of mouse oocytes produced by these OSCs and showed that they can be fertilized to produce healthy embryos. And we identified and characterized an equivalent population of oocyte-producing stem cells isolated from adult human ovaries."

Among the many potential clinical applications for these findings that Tilly's team is currently exploring are the establishment of human OSC banks -- since these cells, unlike human oocytes, can be frozen and thawed without damage -- the identification of hormones and factors that accelerate the formation of oocytes from human OSCs, the development of mature human oocytes from OSCs for in vitro fertilization, and other approaches to improve the outcomes of IVF and other infertility treatments.

Tilly notes that an essential part of his group's accomplishment was collaboration with study co-author Yasushi Takai, MD, PhD, a former MGH research fellow on Tilly's team and now a faculty member at Saitama Medical University in Japan. Working with his clinical colleagues at Saitama, Takai was able to provide healthy ovarian tissue from consenting patients undergoing sex reassignment surgery, many in their 20s and early 30s. Co-lead authors of the Nature Medicine report are Yvonne White, PhD, and Dori Woods, PhD, of the Vincent Center for Reproductive Biology at MGH. Additional co-authors are Osamu Ishihara, MD, PhD, and Hiroyuki Seki, MD, PhD, of Saitama Medical University.

The study was supported by a 10-year MERIT Award to Tilly from the National Institute on Aging, a Ruth L. Kirschstein National Research Service Award from the National Institutes of Health, the Henry and Vivian Rosenberg Philanthropic Fund, the Sea Breeze Foundation, and Vincent Memorial Hospital Research Funds.

Imagine, for a moment, that your job title is "colon hygienist". You would have to explain to people that your job involves inserting a tube into somebody's rectum and swishing their colon out with water. It is claimed that flushing out "toxins" and faecal matter during a colonic irrigation, or hydrotherapy, boosts bowel function, improves the skin, and generally makes you feel lighter and brighter. Except, according to a new review published in the American Journal of Family Practice, none of this has been proven.

Researchers at Georgetown University School of Medicine reviewed 20 studies published during the past 10 years and concluded there was little evidence of benefits of colon cleansing (whether by hydrotherapy, or by ingesting strong laxatives). What they did find, however, was a whole range of side-effects, including abdominal pain, nausea, dehydration, electrolyte imbalance (minerals in the blood that help regulate, among other things, neurological function) and kidney and liver failure.

"From what we gathered, this is not a benign process," says Dr Ranit Mishori, the lead author. "It can be quite dangerous. You are inserting tubes and using up to 60 litres of fluid. This could cause punctures in the colon."

Colonic irrigation had been used in the 19th century in hospitals, with some doctors believing that a build-up of toxic undigested waste caused disease (it is thought colon cleansing dates back to ancient Greece and Egypt). But in the early 20th century, because these claims hadn't been proven, the practice fell out of favour. Until the 1990s, that is, when alternative health spas started offering it, and the process gained some high-profile advocates such as Princess Diana.

Those who have had colonic irrigation have reported passing revolting bits of matter, from strings of mucus to rubbery nuggets. "It's just faeces. It would have come out anyway," says Mishori. "The body is designed to get rid of toxins, in urine and faeces. There is no need to assist it."

It sounds like science fiction: a consultant psychiatrist in Ireland proposed last week that mass medication could be used to make us all happier. Dr Moosajee Bhamjee, former Labour party politician (he accidentally won a seat while campaigning for "the protest vote" in Clare in 1992), says his government should add lithium to the water supply.

Lithium doesn't have quite the romantic appeal of the recreational drugs people take in hopes of getting happier. In fact Bhamjee is not offering us mind-altering experiences. This is a pragmatic option, suited to our depressing economic circumstances. Lithium in the taps, he says, can cut the suicide rate.

The substance is used in psychiatric medicine to treat episodes of mania in people with bipolar disorder, also known as manic depression. In the raw, lithium is a soft, silver-white alkali metal, found in some foods and, in some places, the drinking water. Studies have suggested that in places where people get trace amounts of lithium from the taps, the suicide rate is lower than elsewhere.

In 1990, a study in 27 counties in Texas found lower rates of not only suicide but also homicide and rape in those where the drinking water contained lithium. In 2009, research in Japan found lower suicide rates in areas with lithium in the water. Professor Allan Young from Imperial College has called in the British Journal of Psychiatry for more investigation, saying that "the eventual benefits for community mental health may be considerable".

"Keep your mitts off our muffs!" "I love my vagina!" "You've put my chuff in a huff!" These are some of the slogans of the Muff March taking place along London's Harley Street Saturday morning. Its aim? To raise awareness of the increase in gynaecological cosmetic surgery – both on the NHS and in private clinics. The march, which has more than 300 supporters on Facebook, is organised by campaigning group UK Feminista and performance artists The Muffia, who dress up in nude bodysuits decorated with lavish pubic hair.

At its most modest, the Muff March is against the pornography-influenced obsession with removing pubic hair. But it's also about protesting against the sort of surgery that makes you cross your legs. Typical procedures on offer include labiaplasty (trimming or removing the labia) and vaginal rejuvenation (tightening – usually referred to by "designer vagina").

In the US this industry is worth $6.8m (£4.4m). In the UK the latest figures come from a 2009 report in the British Journal of Obstetrics and Gynaecology. It revealed that in 2008 the number of operations increased by 70% compared with the previous year: 1,118 labiaplasty operations on the NHS. (There were 669 in 2007 and 404 in 2006.) And that's just the NHS. The Harley Medical Group reported over 5,000 inquiries about cosmetic gynaecology last year, 65% for labial reduction.

Professor Linda Cardozo of King's College London recently warned of the risks of labiaplasty: permanent scarring, infections, bleeding and irritation. "The private sector is not recorded, audited or regulated. At least if you have it on the NHS you have to go through your GP and that's a gatekeeper." (Although one anonymous blogger writes on the NHS website: "I have flaps of skin everywhere and the whole thing is a total mess. I will never be able to be intimate again.")

I recently heard of a woman GP very concerned by the number of girls in their mid-teens coming to her worried about what their genitals looked like: she thought it was becoming an issue largely because of the fashion for shaving off pubic hair, which made them more self-conscious. Of course, there are rare cases where there is an underlying medical reason for this surgery, but they are just that, extremely rare. A doctor who has treated women seeking labiaplasty told me: "When you examine them, they are completely normal."

Some experts suggest this is a new form of body dysmorphic disorder. Others see it as a depressing but logical extension of the pornification of our culture. As it becomes more acceptable for young people to watch porn (where a "standardised" genital appearance is encouraged and many of the women have no pubic hair), so young women having their first sexual experiences are being measuring – and measuring themselves – against this weird porn "norm". As one woman who has sought surgery says: "I browsed through one of my brother's Playboys to see what the girls looked like. Some seemed to have very small or almost no labia." In a world where not even your labia can ever be pretty enough, it's time to fight back. Forward march, muffs!

A funny thing happened the other day when I was playing football. I went into a challenge with a player and he came out with a chipped tooth. Apparently my elbow was raised. He kindly suggested it was 50-50. The thing is he works with me. You could say he was a younger, fitter, better version of myself – a new Porsche with multi-media trimmings, compared to my battered Cortina.

When we arrived back at the office, I got a lot of funny looks from colleagues. "You broke Patrick's tooth!" "What could have possessed you?" There was even a rumour that I'd broken his jaw. My colleagues made the connection between our line of work, his relative youth etc.

I have always prided myself on my lack of competitiveness; my easy-going insouciance. Now as I approach my 50th year I am beginning to realise what a sadly misguided git I have been. Even this pride at my lack of competitiveness has been competitive.

American psychologists at the University of Oregon have just revealed that between the ages of 45 and 54, men are at their most competitive. Participants in its research were asked to solve maths equations, either on their own or head-to-head with a rival. Nearly 70% of those aged between 45 and 54 wanted an opponent, whereas only half of those between 25 and 34 did. It all makes a horrible kind of sense. The older, balder, fatter, stupider and more irrelevant we become, the more we feel we have to prove ourselves. So, I run obsessively, stare at my belly, imagining it morphing into a six-pack. When I go out with younger colleagues, there's a bit of me that wants to be able to drink more and vomit less than they do. Even my daughters have told me that these days I have to have the last word in arguments. I find myself going through homework, and saying appalling things such as, "What do teachers know? Listen to me", when I don't have a clue.

Still, at least I'm not as bad as my boss, who does a marine-style lunchtime workout. When he sees me going for a run, he smirks, calls me a sissy and says he's doing real exercise. Pathetic. I'd never do that. Well, not since the time I tried it out, found I was weaker in every respect, and vowed never to be seen in shorts with him again.

The less we can achieve it, the more we want it

Bald, hunched, paunchy, a stranger to buff, grimly aware that my sexual peak was 31 years ago, outpaced by my six-year-old daughter when we run for the bus, I know that, at 49, I'm on the down slope of the sagathon that's going to end in death. So it is good to know that that shows there is one area in which I haven't yet peaked. Previous research has shown that, as cortisol and testosterone decline, so do cognitive performance levels. But here's the twist.Older men want it more even if they can achieve it less - this explains why Ivan Lendl returned to tennis, and why I got embroiled in an incident at the pool last week. I was in the slow lane when I overtook an irritatingly lithe twentysomething swimmer who'd been lapping me. Breathless and smug, I lounged poolside on the edge of the pool - only to realise she'd stopped mid-length to chat to her personal trainer before resuming her implacable progress, damn her.

This incident confirms a gender split: : according to the report, men compete without regard for the likelihood of winning, whereas women don't compete if they can't win. I recognise this: women are more rational than men, though, because I'm competitive, I can't accept that truth.

Is this because men are happy only when asserting their dominance? Perhaps. There is something chilling - isn't there? - in Bernie Ecclestone's eyes when he appears in the shadow of his latest love, as if he's saying: don't you wish your girlfriend was hot like mine? To which the competitive male reply is to change the rules: don't you wish you were two feet taller like me? Nobody said male competitiveness was a beautiful thing.

If we can't win, but we can tilt the playing field until we do. That way we can be winners even as we mutate into the seven loser dwarfs of our declining years – wheezy, paunchy, hunchy, baldy, grumpy, squinty, and surly.
